Alta Vista Walkathon Permission Slip
Alta Vista's 2022 Mummy Mash Walkathon will begin during the school day on Friday, October 28th. The fun will continue after school and conclude at 5:00. 

In order for students to remain on campus for the afternoon festivities, parents/guardians will need to complete this permission slip. Students will not be able to participate in the Walkathon after school without a completed permission slip. Please fill out a separate permission slip for each child.

We hope you'll be able to join us for this fall celebration. 

** TK and Kindergarten students of not need to fill out the form, since they will be dismissed to their guardians earlier in the day and will return with their guardians for the afternoon fun.

* Students will not be signing in and out during this event. Parents/guardians are responsible to letting their child know when they are allowed to leave campus.

* Parents are responsible for providing transportation for their child at the end of the event, no later than 5:00.

* Students are expected to follow all of the Alta Vista rules. If a student is not following the rules, the parent will be called and the student will need to be immediately picked up from school.

* The school and district are not liable/responsible for children who are unattended during the after school portion of the Mummy Mash Walkathon.
